WORKSHOP

Accident investigation

How can an incident be transformed into a genuine opportunity for collective learning in workplace safety? The “Accident Investigation” workshop immerses your teams in a realistic field scenario, placing them at the scene of a reconstructed accident that mirrors situations commonly encountered in professional environments.

The objective is to foster a profound awareness of risky behaviours, hazardous routines, and perceptual errors that can lead to serious accidents. Through an immersive and participatory approach, your employees will enhance their ability to thoroughly analyse incidents, identify root causes, and develop concrete improvement strategies to strengthen prevention measures.

This workshop is an integral part of a comprehensive strategy for managing occupational risks, promoting health and safety at work, and cultivating an active and shared safety culture.

Objective

Develop accident cause analysis skills to implement appropriate prevention measures.

Content

  • Immersion in a simulated accident scene
  • Topics covered: first aid procedures, accident cause analysis, selection of appropriate prevention measures
  • Introduction to cause analysis tools

Format & Target Audience

Available in both in-person and remote formats (online version led by a certified VILT – Virtual Instructor-Led Training coach).

Designed for in-person facilitation with groups of 6 to 12 participants to encourage interaction, engagement, and collective analysis.

Suitable for all employees and management levels.

Can be conducted on your premises, in a training facility, or at a pilot site.

Learning Objectives

  • Develop analytical skills to move beyond surface-level observations and understand the sequence of causes leading to incidents.
  • Enhance individual and collective vigilance by learning to detect weak signals and deviations from protocols.
  • Understand the impact of habits and automatisms by identifying routine behaviours that undermine prevention efforts.
  • Raise awareness of root cause analysis to prevent recurrence of incidents.
  • Encourage open communication about risky situations and promote the right to alert and withdraw from unsafe conditions.

Teaching Methods

The workshop employs active and participatory learning methods, focusing on hands-on experience, field analysis, and knowledge sharing. Techniques include:

  • Root cause analysis tools (e.g., the “5 Whys” method, simplified cause-and-effect diagrams).
  • Role-playing and simulations.
  • Practical guides to integrate into regular safety routines.

Facilitation is provided by an experienced risk prevention consultant skilled in guiding discussions with empathy and rigor, ensuring emotional safety during error analysis.

Workshop Programme

1Immersion in a Simulated Accident Scene

Participants are introduced to a realistic reconstruction of a workplace accident (e.g., fall, injury, technical malfunction). This scenario serves as the starting point for collective analysis.

2First Aid Refresher (Optional)

Depending on participants’ proficiency, a module reviewing essential first aid procedures can be included to ensure effective response to victims and stabilization of critical situations pending emergency services.

3Collective Investigation of Accident Causes

Divided into small groups, participants conduct a thorough investigation: observing the scene, collecting evidence, and engaging in cross-questioning. This phase aims to move beyond quick judgments towards a systemic analysis of underlying causes.

4Feedback Session with the "Victim"

An individual role-plays the victim, sharing their feelings, beliefs, and misunderstandings. This exchange humanizes the analysis and highlights organizational shortcomings, cognitive biases, and human factors contributing to the incident.

5Sharing Insights and Co-Developing Best Practices

Teams present their findings, identify areas for improvement, and collaboratively develop an action plan. This segment emphasizes risk prevention through collective learning and commitment to continuous improvement.
